Job Description:
Lutheran Social Services of Wisconsin and Upper Michigan (LSS)
seeks a dedicated Case Coordinator to join our team of
compassionate professionals. In this role, you will support a
variety of programs designed to foster health, healing, and hope
within the communities we serve. The position involves managing
case loads, developing service plans, and working closely with
individuals and families in need. At LSS, you will contribute to
our mission of assisting people affected by challenges such as
homelessness, addiction, and mental health issues, ensuring they
receive the highest quality of service and support. Conduct
assessments to determine client needs and service requirements.
Develop, implement, and review service plans in collaboration with
clients and other professionals. Monitor progress of service
delivery and make necessary adjustments. Maintain accurate records
and report on case status. Liaise with community agencies and
resources to support client needs. Provide crisis intervention as
needed. Participate in training and development sessions to
continue professional growth. Strong interpersonal and
communication skills Ability to handle sensitive information with
confidentiality Well-organized and detail-oriented Capable of
working under pressure and meeting deadlines Proficient in case
management software Medical Insurance Dental Insurance Vision
Insurance 401k Certified Social Work Case Manager (C-SWCM)
preferred.
