As a Case Coordinator, you will play a critical role in facilitating and managing the services provided to individuals and families in our various programs, including child and family services, addiction support, and community living assistance. Assess client needs and develop service plans. Coordinate and monitor services provided by staff and external partners. Maintain accurate and comprehensive client records. Act as a liaison among clients, families, and service providers. Evaluate program effectiveness and suggest improvements. Facilitate team meetings and training sessions. Ensure compliance with federal, state, and local regulations. Strong interpersonal and communication skills Proficiency in case management software Ability to work collaboratively in a team-oriented environment Strong organizational and multitasking skills Ability to handle sensitive information confidentially Medical Insurance Dental Insurance Vision Insurance 401k Certified Social Work Case Manager (C-SWCM) preferred
