Job Description:

Crossbridge Hospice seeks a dedicated Community Liaison to join our team, serving as a vital link between our hospice services and the community. This role involves advocating for patient needs, increasing community awareness, and fostering relationships with key healthcare providers and community groups. The ideal candidate will ensure consistent communication and education about the hospice services we provide, enhancing our mission to deliver compassionate, end-of-life care. Establish and maintain relationships with local healthcare providers, community organizations, and other relevant stakeholders. Organize and participate in community events and presentations to educate the public and professional groups about hospice services and benefits. Develop and implement outreach strategies to increase service utilization and community support. Collaborate with healthcare professionals to ensure a continuum of care for patients. Gather feedback from the community to improve services and patient care initiatives. Prepare reports and maintain records of activities and engagements for management. Strong interpersonal and communication skills Ability to work independently and within a team Experience in public speaking and presentations Understanding of healthcare systems and palliative care Project management and event planning skills Certification in Community Relations or Healthcare Administration preferred
